Medical Home Committee
The Medical Home Committee convened the state’s major health care stakeholders, including Medicaid, for the purpose of developing and accelerating the adoption of standard components and criteria for the delivery of health care services via the patient-centered medical home system of care to enhance both quality of care and access to services.
The Patient-Centered Medical Home (PCMH) is an approach to providing comprehensive primary care for children, youth and adults. The PCMH is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family.
Patient-centered medical homes are systems of care led by a team of primary care providers who partner with the patient, the patient’s family and the community to coordinate care in all settings, from specialists and hospitals to pharmacies, nursing homes and home health agencies. The concept of the medical home puts the patient at the center of health care, where teams of providers help the individual to navigate.
