Update I: Security Risk Assessments

11 16 Security risk assessment
Did your medical practice work with the Louisiana Health Information Technology Resource Center to achieve Meaningful Use of Certified EHR Technology? If so, you may have used a template provided by the LHIT Resource Center staff for completion of a Security Risk Assessment (SRA). The template, along with sample policies and an audit log review document, would have been saved as documentation in the event of an audit by the Medicare Contractor for CMS or the Medicaid Contractor for Louisiana Medicaid. In many cases, sample policies were provided for the practice to use as its own. Based on guidance issued by the Office for Civil Rights regarding SRAs, we recommend that you address the areas listed below in the template and in the documented policies and procedures for your practice if you have used or are still using this document:

  • Identify potential threats and vulnerabilities to practice and to ePHI stored in practice
  • Determine likelihood that threat will occur
  • Assess level of impact to practice should threat occur
  • Determine and assign a level of risk for all threat and vulnerability scenarios
  • Address all vulnerabilities and threats by adding action needed for remediation

In March 2014, the Office of the National Coordinator for Health Information Technology (ONC) developed a downloadable SRA Tool, specifically designed to assist providers.
