Click to EnlargeToday's health care system can be confusing and challenging to navigate. The Louisiana Health Care Quality Forum recognizes this and has adopted a model of care that may help you find your way through the system -- the Patient-Centered Medical Home.
 
    As a consumer and patient, you are at the center of the medical home.  A medical home is not a building, house or hospital, but rather an approach that provides you with total health care.  A medical home is defined as primary care that is:
    • accessible
    • continuous
    • comprehensive
    • family-centered
    • coordinated
    • compassionate
    • culturally effective
    In the medical home, a physician works in partnership with you and your family to ensure that your medical and non-medical needs are met. Through this partnership, the health care provider can help you access and coordinate specialty care, educational services, out-of-home care, family support and other public and private community services that are important to your overall health and your famly's health.
