"We look at care coordination and integration across the continuum and enhance the communication."

Today's health care system can be confusing and challenging to navigate. The Louisiana Health Care Quality Forum recognizes this and has adopted a model of care that may help you find your way through the system -- the Patient-Centered Medical Home.
As a consumer and patient, you are at the center of the medical home. A medical home is not a building, house or hospital, but rather an approach that provides you with total health care. A medical home is defined as primary care that is:
- culturally effective
In the medical home, a physician works in partnership with you and your family to ensure that your medical and non-medical needs are met. Through this partnership, the health care provider can help you access and coordinate specialty care, educational services, out-of-home care, family support and other public and private community services that are important to your overall health and your famly's health.
In Louisiana, there are currently 61 Patient-Centered Medical Homes that are recognized by the National Committee on Quality Assurance (NCQA). To view a map that shows the different sites, please click here
