Advance Care Planning and LaPOST
Susan Nelson, MD, chairwoman of the LaPOST Coalition, will be the featured speaker on a webinar about advance care planning. Board certified in internal medicine, geriatrics and hospice and palliative medicine, she serves as Medical Director of Senior Services at Franciscan Missionaries of Our Lady Health System and Franciscan PACE. Dr. Nelson is a fellow of the American Academy of Hospice and Palliative Medicine and the American College of Physicians as well as an advocate for senior health. She works with the Louisiana State Medical Society Council on Public Health and the Louisiana Department of Health and Hospitals. In addition, she is the Vice-Chair of the National Physician Orders for Life Sustaining Treatment (POLST) Paradigm Task Force and its Executive Committee and serves on the Board of Directors for the Louisiana Health Care Quality Forum.

Conversations Change Lives is a web-based, video module that is available 24/7 for your inservice and/or annual training needs. Dr. Susan Nelson, LaPOST Coalition Chair, hosts the four-part series and guides you through each lesson:

- Having The Conversation
- Completing The LaPOST Document
By creating an account with a username and password, you can view the videos, review the key learning concepts and take brief post-tests (approximately 35 minutes). After passing the post-tests, you will be able to print a Certificate of Completion for documentation.
