Advance Care Planning and LaPOST
Talking about goals of care and end-of-life decisions with residents and caregivers in long-term care facilities can be difficult and uncomfortable at times. Still, these conversations are vital for residents, caregivers and the health care team. To support health care professionals, Susan Nelson, MD, will present a series of on-site seminars focusing on different aspects of advance care planning and the Louisiana Physician Orders for Scope of Treatment (LaPOST) document.
Dr. Susan Nelson, LaPOST Coalition Chair, is board-certified in internal medicine, geriatrics and hospice and palliative medicine.
