The Patient-Centered Medical Home (PCMH) model of care emerged during the late 1960s as an approach to providing primary care to children with special needs. Over the years, however, PCMH has evolved into a team-based approach to providing comprehensive, personalized health care to the whole family.

Because it's based on a partnership between patients and their physicians and designed to improve the quality, effectiveness and efficiency of the care provided to the patients, the PCMH model holds tremendous promise as a method of improving the organization and delivery of primary care.

Since 2008, the Quality Forum has focused on advancing this care model in our state, becoming the first statewide multi-stakeholder collaborative in Louisiana to do so. As a leader in our state's efforts to redesign delivery systems to support patient-centered, coordinated care, the organization has developed a foundation of integrated standards and technology to bring universal access to quality primary care to all residents of Louisiana.
