Advance Care Planning and LaPOST for Long-Term Care Facilities

Susan Nelson, MD, LaPOST Coalition Chair, will host this one-hour webinar on Advance Care Planning and the Louisiana Physician Orders for Scope of Treatment (LaPOST) document. Board-certified in internal medicine, geriatrics and hospice and palliative medicine, she serves as Medical Director of Senior Services at Franciscan Missionaries of Our Lady Health System and Franciscan PACE. Dr. Nelson is a fellow of the American Academy of Hospice and Palliative Medicine and the American College of Physicians as well as an advocate for senior health. She works with the Louisiana State Medical Society Council on Public Health and the Louisiana Department of Health. In addition, she is the Vice-Chair of the National Physician Orders for Life-Sustaining Treatment (POLST) Paradigm Task Force and its Executive Committee and serves on the Board of Directors for the Louisiana Health Care Quality Forum.
